Uninsured Services

Although the majority of services provided by your doctor are covered by Alberta Health, some are not. Uninsured services include services performed for third parties (school, work or government departments,) or other services such as administrative services or clinical services not paid for by Alberta Health. These fees are based on the Alberta Medical Association's guidelines for uninsured services and are subject to change.

• sick note/massage note/physio note - $50
• pregnancy leave/employment insurance form - $75
• letter of fitness (camp, sports, daycare, travel) - $50
• Driver's medicals (all ages) - $150
• blue cross special authorization - $50
• disability benefit form - $75 minimum

• phone appointment - $50 minimum
• regular appointment - $50
• procedure - $100 minimum 
• complete/annual physical or intro appointment - $150

• prescription refills without a visit - $60

• pregnancy test - $10
• injections (travel vaccinations such as Hep A and B) - $30 per injection
• Botox for hyperhidrosis (excessive sweating) - $200 - $250 injection fee

• transfer of medical records - $50 minimum

• Botox or Xeomin - $10 per unit
• Cosmetic removal of lesions/moles - $100 per lesion
